    Referral to Our Hospital

    A pre-admission screening is completed for all referred patients by a designated member of the rehabilitation team, and admission is approved by a physician.    

    Care Team Evaluation
    Once admitted, further evaluation is conducted by a care team who will create a custom rehabilitative treatment plan to address your specific condition and recovery needs. 
    Treatment
    An integrated team of doctors, nurses and therapists work together to help you maximize functional independence, find emotional strength and confidence, and  transition back to home, work and the community.

    Initial Evaluation
    You will meet with a physical or occupational/hand therapist who will evaluate your injury or condition, discuss your goals and specific needs and develop an individualized treatment plan.
    Regularly Scheduled Appointments
    You will work with a member of our clinical team who will instruct and supervise your care. As you reach specific treatment goals, you will progress to new levels, treatments and exercises. 
    Home Exercise
    You may be provided with a home exercise program that complements your plan of care.

    Transfer from ICU
    Admission to our critical illness recovery hospitals is carefully orchestrated through our admissions team who take steps to ensure patients' needs are met and a smooth transition occurs.
    The First 24 Hours
    Shortly after transfer, a full assessment of the patient is conducted by a physician-led team of specialized health care professionals who outline a customized care plan.
    Ongoing Care and Recovery
    While under our care, patients will continue their recovery supported by daily physician rounds, 24-hour respiratory care, as well as physical, occupational and speech therapy to promote functional independence
